Agency Overview:
DISC Village, a non-profit community-based agency, has been in operation for over 50 years and is one of the largest and most comprehensive prevention, intervention and treatment agencies in Florida. Through the years, DISC Village has become recognized as a leader in the state for initiating, managing and coordinating substance abuse prevention/intervention, family welfare and delinquency programs in Leon County and North Florida.
We are seeking a full-time Outpatient Counselor for our Leon County office. The Counselor will conduct assessments, provide group and individual counseling, and perform intervention services in an outpatient setting, working primarily with the youth population.
Essential Responsibilities:
- Complete comprehensive, individualized Biopsychosocial assessments and additional Medicaid and/or SAMHSA required documentation.
- Document assessment findings and referral recommendations in the clinical notes; including writing progress notes and documenting all delivered services.
- Provide individual, group, and family counseling for mainly youth client and their families. Services to be provided in various settings including but not limited to: in-home, in-school, out of office, and other community locations. Sessions will be appropriately and accurately documented.
- Develop individualized Treatment/Service Plans with clients and families and review the plan at the identified midway point with the client and family.
- Monitor the quality of care/reassess the need for service. Consistently monitoring the progress of the person served to make a determination whether additional services are needed.
- Staff each case with a supervisor or designee and make appropriate clinical recommendations that will minimize future behavioral and affective disorders.
Qualifications:
- Master’s Degree from an accredited college or university with major course work preferably complete in Criminology, Social Science, Mental Health, Psychology, or Chemical Dependency field. Preference of Master’s Degree in noted fields.
- At least two (2) years of experience in individual/family counseling is preferred.
