About Us: DISC Village, Inc. is a private non-profit behavioral health organization that has been serving the Big Bend area of North Florida for over 50 years. We provide expertise in substance abuse, mental health, and child welfare services to adults, adolescents, and families.

Program Overview: Our Dependency Case Management Services aim to deliver the highest quality of child welfare prevention and intervention services to children and their families within their communities. Our program offers a continuum of services to assist families with acute or chronic conditions that threaten child safety and family stability.

Essential Responsibilities:

  • Supervise families and children in cases where abuse, abandonment or neglect allegations are validated by Child Protective Investigations. The Dependency Case Manager makes ongoing assessments regarding child safety and well-being of all children under supervision.
  • Complete initial case assessments/social history/risk assessments within 15 days of assignment of the case. Complete updated assessments every five months or upon a substantial change in the family composition or dynamics.
  • Visit each client/family on a monthly or more frequent basis to monitor compliance with and evaluate the effectiveness of services and providers, counsel with family members to support their efforts to alleviate risk, maintain telephone contact with the client/family as needed.
  • Make telephone, written and face to face contacts with providers or others to assess effectiveness of or compliance with services. Be available, as assigned, to take calls and walk-in inquiries when on call.
  • Discuss referrals, goals, plans, and barriers with the client/family, involve the client/family in the development of a workable case plan and utilize techniques of determination of client/family strengths and needs in order to develop the case plan.
  • Assist in presentation to the Court, appear at arraignment, disposition, judicial review, and other court hearings as scheduled, transport or arrange transportation for client/family to appear in Court, if required.
  • Conduct home studies or initiate Interstate Compact for the Placement of Children (ICPC) or out-of-county inquiries to locate alternative placement possibilities for children who cannot remain in their own homes. This includes courtesy requests from other counties for home studies and incoming ICPC requests for home studies.
  • Assist the client/family in making alternative placements for children in the home. This may involve studying relative/non-relative homes or completing applications and attending appropriate staffings for group home or treatment facility placements.
  • Document investigations of abuse, abandonment or neglect on active cases and cooperate in any such investigation undertaken by Child Protective Investigators.
  • Perform other related duties, as required.


Qualifications:

  • Bachelor’s degree from an accredited college or university in social work, psychology, mental health, or a related field
  • Obtain Child Welfare Certification within the probationary period and maintain it on an ongoing basis
  • Ability to be sensitive to the service population’s cultural and socioeconomic characteristics
  • Must be able to work under tight schedules and handle the pressure associated with deadlines
  • Valid Florida state-issued driver’s license and insurance required
  • Must maintain a professional demeanor, respect for authority, appropriate interaction skills, and confidentiality standards
  • Excellent verbal and written communication skills

Base Salary: $ 45,011.20 At Hire, $47,507.20 After Passing Pre-Service Exam, $50,003.20 Once Certification Obtained
