Overview
Discovery Behavioral Healthcare is looking for a PHP Coordinator to run a grant based pilot program who will assess, diagnose and provide treatment to individuals struggling with substance use disorders; through individual and group counseling, education and case management, aiming to help clients achieve sustained recovery.
This is a 1 year grant based position, with the possibility of becoming permanent. This position has benefits provided by the UFCW3000.
Duties
- Provide screening and evaluation of individuals and refer them as needed to treatment/service programs as are available in the agency and in the community.
- Prepare assessments (including DUI and deferred prosecution) , and deferred prosecution evaluations and treatment plans.
- Lead discussions and present lectures/films for group sessions.
- Maintain records of client participation and outcomes in the partial hospitalization program.
- Engage in community outreach to educate on the partial hospitalization program.

Requirements
- Completion of an accredited addiction studies curriculum.
- Must be registered with DOH as and SUDP plus have two (2) years experience.
