Job Duties:
- Work cooperatively with all persons relative to residential services including Service Coordinators, BSP's, Human Resources, Quality Assurance and Director of Compliance.
- Collect weekly schedules and post in each ISS (24-hour) home.
- Provide training to Person Centered Support Staff as instructed by Director of Residential Services and/or Regional Residential Supervisor.
- Ensure all safety concerns are reported immediately to supervisor
- Provide assistance to Person Centered Support Staff during behavioral episodes when possible.
- Meet with Regional Residential Supervisor at least weekly to review specific needs for each consumer.
- Participate on Safety Committee and/or other committees as requested.
- Ensure required blank forms for all documentation are in the home and available in an appropriate quantity and prior to ever running out (mileage, billing sheets, progress notes, medical logs, etc).
- Review the documentation in each home weekly for accuracy and ensure all corrections are made before submitting them for billing (notes dated and signed for each shift, each day). Read the progress notes for each client at least weekly to ensure that any communication of needs/concerns are addressed in a timely manner. Review for accuracy and completeness of every note...appropriate billing codes identified on each note, signatures, times, dates, etc). Your signature on anything verifies that you have reviewed the information for accuracy. Submit to supervisor for final approval after you have reviewed and authorized. Make sure that the weekly home documentation is filed in the client ISS program book after reviewed and approved by the BSP/Supervisor. Make sure items needed for billing are submitted to BSP for final approval and submission to payroll department.
- Ensure that consumer mileage is used in an appropriate manners and that it does not exceed allowable amount.
- Have readily available a list of people and their availability that have completed DD6 training for the client who needs coverage and start covering shifts with people that have the fewest hours first for coverage.
- Provide information to Residential Supervisor & Residential Director for annual evaluations and personnel functions.
- Serve as the point of contact for on-call needs/call-offs/staffing needs regularly, with the exception of the weekend on-call schedule when it is your weekend to not be on call. Document all calls received: purpose for call, outcome and next steps. All Calls!
- Conduct and document quarterly fire drills in each home.
- Make frequent check to homes to ensure needs are met.
- Ensure a menu is in place, grocery shopping occurs weekly that covers all items needed for menu and that food needs are met. (BSP should have programs in place that include preparing a menu, completing a grocery list and grocery shopping).
- Ensure non-food items are in stock at the home (toilet paper, paper towels, laundry detergent, cleaning supplies, etc).
- Ensure any bills that come to the home are submitted to the Service Coordinator for submission to the payee ASAP to ensure all bills are paid.
- Ensure that everything in each home is in good working order (plumbing, appliances, heating/cooling, electric, cable, telephone, television, etc). Notify the Service Coordinator if anything is needed from the landlord so that the SC can follow up. Continue notification until the issue is resolved.
- Ensure that all consumers have weather appropriate, clean and well-fitting clothing/shoes and inform the SC if anything is needed.
- Ensure that consumer has all necessary items needed for independent living and that all items are clean and in good working order (furniture including couches/seating, table and chairs for dining, be and bedding, linens, towels, dish towels, etc), cookware, curtains/blinds/drapes, etc and any other household items.
- Ensure any other needs/concerns, guardian/consumer requests are reported and follow up to be sure the needs have been met.
- Complete household inventory for each consumer as part of intake, when moving, when changing roommates, annually or upon request.
- Follow the schedule for laundry days and as needed for specific consumers. Ensure that each consumer has a laundry check each week to pay for laundry services.
- Ensure that a set of keys are provided to Administration for each ISS home to be kept for emergencies.
- Inform supervisor of staffing needs ASAP so that the training and development manager can recruit new employees.
- Participate in consumer meetings as needed.
Position Requirements:
- High school degree/GED required
- 18 years of age or older
- Valid driver's license
- Ability to work in a home setting
- Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
- Ability to handle physical aspects of job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time
- Must maintain state regulatory certifications, as required by state/program requirements
