Diversified Treatment Alternative Centers is searching for a top talent Caseworker in Williamsport, PA-for our Partial Hospitalization Program! The Caseworker is responsible for the facilitation of therapy in a variety of specific identified areas including the development and implementation of an individualized treatment plan, documentation of therapeutic experience via progress notes and communication with the partial team consisting of, Mental Health Professional, Program Director, and Program Supervisor to provide an atmosphere conducive to wellness.
Hours: 7 am- 3:30 pm and NO weekends.
- Payrate Increased! $20.00- $23.00 phr!
Qualifications:
- Bachelor’s degree. (Please apply with confidence, an immediate interview is desirable if you meet the necessary criteria.)
- Training and Experience: Sufficient previous counseling experience, preferably with adolescents.
What will you do?
- Develop and implement a treatment plan that addresses the needs of the patient with input from the patient, referral source, and Program Treatment Team. All goals/objectives are to be realistic, obtainable, and measurable.
- Conduct individual counseling sessions with primary clients on a weekly basis.
- Facilitate group therapy sessions focused on topics such as family, socialization skills, relationship development, self-worth, problem-solving, etc.
- Provide for the safety, security, and appropriate social interaction and social behavior of all youth in the program.
- Facilitate on-going family sessions involving and maintain appropriate communication with significant others and agencies.
- Present didactic education presentation to patient, community, and co-workers as delegated by the Program Supervisor and/or Program Director.
- Maintain professional documentation duties, i.e. court reports, taskwork, discharge summaries, individual and family notes, log entries, incident reports, progress notes, treatment plans and TPRs and clinical testing.
- Demonstrate a standard of conduct that provides effective role modeling for patient population and peer group.
- Present accurate clinical information to referral sources, family members, and treatment team members.
- Participate in updating the Treatment Plan every 20 days with the entire treatment team.
- Plan and implement therapeutic, recreational, and educational activities daily.
- Facilitate appropriate discipline through therapeutic intervention as needed.
- Attend weekly staff meetings and prepare required case manager information to the treatment team.
- Improve level of competence through participation in on-going supervision and attend on- and off-site training seminars.
- Document and maintain client data via EMR.
- Perform all other duties assigned by the Program Supervisor and Program Director to provide an atmosphere conducive to wellness.
You must be at least 21 years old. Ability to obtain Act 33 clearances, Criminal History and FBI Act 73 clearance. A valid driver's license is required. Ability to pass Safe Crisis Management is also required.
