Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring effective discharge planning, and managing the overall health needs of patients across various settings. This position requires strong clinical skills, excellent communication abilities, and a commitment to providing high-quality patient care.

Duties

  • Assess and monitor patients' vital signs and overall health status.
  • Collaborate with interdisciplinary teams in Level I and Level II Trauma Centers to develop and implement individualized care plans.
  • Facilitate discharge planning to ensure smooth transitions for patients leaving acute care settings.
  • Provide comprehensive patient care, including behavioral health support and infectious disease management.
  • Engage in utilization management to optimize resource use while maintaining quality care standards.
  • Coordinate with ICU teams for critical patient management and follow-up care.
  • Educate patients and families about health conditions, treatment options, and post-discharge resources.
  • Maintain accurate documentation of patient interactions, care plans, and progress notes.

Skills

  • Proficient in assessing vital signs and interpreting clinical data.
  • Experience working in Level I and Level II Trauma Centers preferred.
  • Strong background in discharge planning processes.
  • Knowledgeable in patient care practices across various healthcare settings, including acute care and ICU environments.
  • Familiarity with behavioral health issues and infectious disease protocols.
  • Excellent organizational skills with the ability to manage multiple cases effectively.
  • Strong interpersonal skills for effective communication with patients, families, and healthcare professionals.
  • Ability to work collaboratively within a multidisciplinary team to enhance patient outcomes.

Pay: $72,793.00 - $76,649.00 per year
