We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Duties
- Assess and monitor patients' vital signs and overall health status.
- Collaborate with interdisciplinary teams in Level I and Level II Trauma Centers to develop and implement individualized care plans.
- Facilitate discharge planning to ensure smooth transitions for patients leaving acute care settings.
- Provide comprehensive patient care, including behavioral health support and infectious disease management.
- Engage in utilization management to optimize resource use while maintaining quality care standards.
- Coordinate with ICU teams for critical patient management and follow-up care.
- Educate patients and families about health conditions, treatment options, and post-discharge resources.
- Maintain accurate documentation of patient interactions, care plans, and progress notes.
Skills
- Proficient in assessing vital signs and interpreting clinical data.
- Experience working in Level I and Level II Trauma Centers preferred.
- Strong background in discharge planning processes.
- Knowledgeable in patient care practices across various healthcare settings, including acute care and ICU environments.
- Familiarity with behavioral health issues and infectious disease protocols.
- Excellent organizational skills with the ability to manage multiple cases effectively.
