Position Purpose:
The Intensive In-Home (IIH) Qualified Professional is responsible for coordinating and delivering care to individuals and families experiencing mental health, substance abuse, or developmental disability challenges. This role ensures the seamless movement across levels of care by providing direct support and facilitating access to essential resources.
Location: Greenville
Hours: Part-time, hours will vary and flexible.

Program Summary:
Intensive In-Home services involve both direct and indirect interventions, including service coordination, case management, and crisis response. These services are delivered in diverse community settings, such as homes, schools, shelters, libraries, and other locations, to ensure accessibility and effectiveness. Additional support includes phone consultations and coordination with individuals assisting the recipient in achieving their goals outlined in the Person-Centered Plan (PCP).
Essential Duties and Responsibilities
- Provide intensive case management, therapy, crisis intervention, and skill development for children and families.
- Deliver services in community-based settings with a focus on family and youth engagement.
- Maintain a minimum of 60% face-to-face contact with clients and families.
- Ensure at least 60% of work hours are spent outside the agency facility.
- Assist in crisis de-escalation, assessment, and intervention to prevent hospitalizations and out-of-home placements.
- Link individuals and families to community resources and services.
- Provide parenting skills training to improve family functioning and support behavioral management.
- Monitor psychiatric symptoms and substance use disorders, offering interventions as needed.
- Train caregivers in implementing home-based behavioral supports.
- Maintain proper documentation of services provided, ensuring compliance with agency and regulatory requirements.
- Serve as a "first responder" for crisis situations, ensuring 24/7 availability for individuals receiving Intensive In-Home Services.
- Participate in required training, including Medication Administration, Nonviolent Crisis Intervention (NCI), and Community First Aid/CPR within the first 90 days of employment.
Qualifications, Education, and Experience
All Intensive In-Home service providers must have at least one year of documented experience working with the target population.
Qualified Professional (QP) Criteria
A QP within the Mental Health/Developmental Disabilities/Substance Abuse (MH/DD/SA) system of care must meet one of the following criteria:
- (a) Licensed or certified by a governing board regulating a human services profession (except registered nurses) and has four years of full-time MH/DD/SA experience with the target population.
- (b) Master's degree in a human services field with one year of post-graduate MH/DD/SA experience, or a substance abuse professional with one year of supervised experience in alcohol and drug counseling.
- (c) Bachelor's degree in a human services field with two years of post-bachelor’s MH/DD/SA experience, or a substance abuse professional with two years of supervised experience in alcohol and drug counseling.
- (d) Bachelor's degree in a non-human services field with four years of post-bachelor’s MH/DD/SA experience, or a substance abuse professional with four years of supervised experience in alcohol and drug counseling.
Working Conditions and Physical Demands
- Work is primarily conducted in community settings, including homes, schools, and shelters.
- The role requires extensive travel to meet with clients and provide in-person support.
- The position may involve responding to crisis situations outside of regular business hours.
