Job Description:
Yotaron Enrichment Resource Center is looking for an Intensive In Home Qualified Professional (QP) Under the supervision of the Intensive In Home Therapist, the IIH Qualified Professional (QP) will provide intensive in home and Case Management services to students within the intensive in home team with case management supports for the family and youth, behavioral implementations at the direction of the IIH Therapist, Individual and family work with the 12 families assigned to the IIH team.

Requirements:
Major Responsibilities:
- Provide behavioral support to students and families within the community and home environment.
- Work with team lead and other team member to coordinate schedules and provide required visits for each student and family.
- Implement Eliada Intensive In Home philosophy/model to teach behavioral and social skills that contribute to the student remaining in their home or community-based placement
- Utilize the agency’s crisis intervention model
- Ensure case management services are provided for each student.
- Assess each beneficiary’s needs for comprehensive services.
- Develop and implementing the PCP.
- Convene Child and Family Team meetings to coordinate the provision of multiple services and the development of, and revisions to, the PCP.
- Link the beneficiary or family to needed services and support.
- Monitor the provision of services and supports. Assessing the outcomes of services and supports.
- Collaborate with other medical and treatment providers.
- Collaborate with other behavioral health service providers, as well as with identified medical (including primary care and psychiatric) and non-medical providers (e.g. Dept. of Social Services, school, Dept. of Juvenile Justice), engages community and natural supports, and includes their input in the person-centered planning process.
Qualifications:
- Bachelors level degree in human services with 2 years post graduate field work with children and adolescents
- Bachelors level degree in any field of study with 4 years post graduate field work with children and adolescents
- Masters level degree in human services with 1 year post graduate field work with children and adolescents
- Masters level degree in any field of study with 2 years post graduate field work with children and adolescents
