Key Responsibilities:

Breast Cancer Navigation:
- Provide comprehensive education and resources to patients and their families regarding all aspects of breast cancer care, including diagnosis, treatment options (surgery, chemotherapy, radiation, hormonal therapy), and available support services.
- Guide patients through the complex medical system, including scheduling appointments, coordinating care with multiple providers, and ensuring timely access to necessary services.

Barrier Reduction:
- Identify and address potential barriers to care, such as transportation, insurance, financial constraints, and language barriers.
- Advocate for patients' needs and ensure access to quality breast cancer care.

Emotional Support:
- Offer emotional and practical support to patients and their families throughout the treatment journey, including coping with diagnosis, managing treatment side effects, and navigating the emotional impact of breast cancer.

Care Coordination:
- Facilitate referrals to appropriate specialists (e.g., surgeons, oncologists, radiologists, genetic counselors) and community resources, such as support groups, financial assistance programs, and rehabilitation services.

Patient Empowerment:
- Empower patients to make informed decisions about their care by providing clear and understandable information and supporting their active participation in their treatment plan.

- Utilize patient navigation tools to track patient progress, identify trends, and evaluate the effectiveness of navigation services for breast cancer patients.
Qualifications:
- High School Diploma or GED
- Bilingual fluency in Chinese and English
- Two years of experience in a medical office or related healthcare setting
- Certified Patient Navigator certification preferred
- Experience working with breast cancer patients
- Knowledge of breast cancer treatment guidelines and resources
- Bachelor's degree in a healthcare-related field preferred (e.g., healthcare administration, social work)
- Excellent communication, interpersonal, and organizational skills
- Strong problem-solving and critical thinking abilities
- Ability to build rapport and establish trusting relationships with patients
- Strong empathy and compassion
- Proficiency in using electronic health records (EHR) and other relevant software
