Position Summary :
The Breast Health Patient Navigator plays a vital role in supporting individuals diagnosed with breast cancer throughout their entire journey. This position requires a compassionate and highly organized individual with strong communication skills and a deep understanding of the breast cancer care continuum. The ideal candidate will be bilingual in Chinese and English, enabling them to effectively serve a diverse patient population.
Key Responsibilities :
- Breast Cancer Navigation :
Provide comprehensive education and resources to patients and their families regarding all aspects of breast cancer care, including diagnosis, treatment options (surgery, chemotherapy, radiation, hormonal therapy), and available support services.
Identify and address potential barriers to care, such as transportation, insurance, financial constraints, and language barriers.
Offer emotional and practical support to patients and their families throughout the treatment journey, including coping with diagnosis, managing treatment side effects, and navigating the emotional impact of breast cancer.
Facilitate referrals to appropriate specialists (e.g., surgeons, oncologists, radiologists, genetic counselors) and community resources, such as support groups, financial assistance programs, and rehabilitation services.
Empower patients to make informed decisions about their care by providing clear and understandable information and supporting their active participation in their treatment plan.
Utilize patient navigation tools to track patient progress, identify trends, and evaluate the effectiveness of navigation services for breast cancer patients.
