Registered Nurse (RN) Case Manager – Home Health
Position Overview:
As a Registered Nurse (RN) Case Manager in Home Health, you will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care in accordance with physician orders and the established plan of care. Under the supervision of the Director of Clinical Services or Executive Director, you will play a vital role in patient education, family and caregiver support, and interdisciplinary collaboration.
This position involves:
- Managing patient care through comprehensive assessments, planning, and implementation of clinical services.
- Communicating with physicians, therapists, and other healthcare providers to coordinate the best possible patient outcomes.
- Supervising and providing guidance to Licensed Vocational Nurses (LVNs/LPNs) and Home Health Aides (HHAs).
- Ensuring compliance with Quality Assurance and Performance Improvement (QAPI) standards.
Qualifications & Requirements
- Licensure: Active RN license in the state of practice
- Experience: Minimum of two (2) years of nursing experience (Home Health preferred)
- Certifications: CPR certification (American Heart Association or American Red Cross)
- Skills & Abilities:
- Strong clinical judgment and ability to work independently in a home health setting
- Excellent verbal and written communication skills for patient and caregiver education
- Proficiency in electronic health records (EHR) documentation
- Commitment to compassionate, patient-focused care
- Additional Requirements:
- Valid driver’s license, auto liability insurance, and reliable transportation
- Availability for on-call rotations (at least once per month)
- Willingness to travel up to 100% to patient homes
Education Requirements
- Graduate of an accredited school of professional nursing
- High School Diploma or GED required
Work Environment & Physical Requirements
- Provides care in patient homes and agency office settings
- Requires physical activity such as kneeling, squatting, reaching, twisting, and lifting (up to 100 lbs with assistance)
- Must tolerate exposure to varying home environments, bodily fluids, and hazardous materials
- Must meet all state-mandated health requirements, including an annual TB test
Schedule & Compensation
- Schedule: Monday – Friday (8:30 AM – 5:00 PM)
- Compensation: Salary or Pay Per Visit (PPV)
