Case Manager (District Attorney's Office)Employment Type : 0 - Full-time Regular Pay Range : $58,835.00 - 88,253.00Location : Castle Rock, COOvertime Exempt : Y Elected Office / Department : DISTRICT ATTORNEYProvides a variety of support to aidin participant's successful completion of the diversion program including : coordinating both internal and external services, identifying and fosteringprofessional relationships with other community-based providers, connectingdiversion participants with community resources and providers as needed,conducting multi-disciplinary meetings, providing educational advocacy,maintaining client records by documenting all participant contacts and trackingprogress, and facilitating restorative justice practices through the DistrictAttorney's Office. May assist with data collection and reporting relatedto grant reports and applications. The Diversion Case Manager may work with clients with a range of needsincluding those needing high acuity mental health navigation, youth chargedwith sex offenses, and low risk clients needing to engage with restorativejustice practices. Generally, the hiring range is $58,835-$73,544 annually. Positions allocated to this class may notperform all of the duties listed) : Coordinates with the diversion teamto identify appropriate restorative justice practices for participants andfacilitates restorative justice practices including community circles.Collaborates with clinical staff to create individual plans outlining programrequirements for diversion participants. Works in conjunction withprogram participants to promote successful completion of their individualizedprogram goals. Coordinates shared treatment planning with externaltreatment providers, executes appropriate waivers and releases of information,and ensures compliance with treatment and program requirements. Leadson-going psychoeducation groups for the lowest-need program participants incollaboration with a licensed therapist and interns. Researches and identifiescommunity-based resources as needed, conducts due diligence on community-basedresources and establishes professional working relationships between agenciesto foster ease of access and collaboration. Conducts multi-disciplinarymeetings. Helps families to understand educational options and coordinates withschools on transitions. Creates case management notes in participant electronicfiles and all other required paperwork or documentation, maintains participantconfidentiality, tracks participant progress and helps to determine programviolations and successful completion. Works closely with the clinical team toensure appropriate level of supervision for each client, determines frequencyand modality of participants contacts with the program. Educates client andfamily regarding the justice system and consequences of further lawinvolvement. May assist in tracking and collection of restitution.Collects and reports various case-related statistics. May assist with data collection and reporting related to grant reports andapplications. Formulates case conceptualization and case presentations to supervisor, co-workersand outside agencies. Responds to clients in crisis and makes referrals asneeded. Attends mandatory staff meetings and bi-annual retreats, variousconferences and professional training seminars. Presents material about programservices and / or relevant topics to schools and community service agencies. Mayserve on one or more community service / agency committees. SUPERVISION RECEIVED / EXERCISED : Works under the supervision and direction of the JuvenileDiversion Supervisor and Director unless otherwise assigned. Receives caseassignments through a screening, assessing and team staffing process. May workwith interns. May help orient new employees.SCOPE OF INTERPERSONAL CONTACTS : Contacts are with supervisors, co-workers, programparticipants, attorneys, court officials, community treatment providers, otherrelated agencies' personnel and the general public. MINIMUM QUALIFICATIONS : EDUCATION / EXPERIENCE : Bachelor's degree, or equivalentexperience, in social work or closely related human services field. Two (2)years of progressively responsible case management duties involving directservice provision with adults, juveniles and families or any equivalentcombination of education and experience preferred. Experience facilitatinggroups or collaborative meetings and familiarity with handling large caseloads(40 ). Bi-lingual (Spanish-English) preferred. REQUIRED SKILLS, KNOWLEDGE, ANDABILITIES : Familiarity with the process of casemanagement and an interest in the criminal and juvenile justice systems andtrends. Ability to coordinate with community agencies / resources,facilitate meetings with outside providers on behalf of the client, understandand explain program components to clients, families and outside agencies,identify and analyze problems, make recommendations for effective solutions,establish and maintain positive working relationships. Ability to hold othersaccountable in a supportive way. Effective oral and written communication,ability to multi-task, self-driven and accountable to supervision. Works as amember of a high-functioning, self-directed team. Exceptional time managementand organizational skills. Knowledge of restorative justice principlesand experience facilitating varied restorative justice practices. WORK ENVIRONMENT : The majority of work is provided in a standard officeenvironment. In the event of an emergency / disaster in or near the County, all County employees are expected to make every effort to be available to assist the County Manager, Elected / Appointed Officials and Department Directors to ensure the continued operation of any and all necessary County functions. This may mean being available to perform additional duties and hours beyond what is normally required. In the event that an exempt employee does work more than 40 hours a week in support of County operations during an emergency, such employee may receive overtime or other appropriate wage compensation in accordance with existing County policies or at the discretion of the County.
