JOB TITLE: Social Worker-Developmental Disabilities Case ManagerDEPARTMENT: Social ServicesSTATUS: Full-time; 37.5 hours/week. STARTING SALARY: The starting salary for this position is $63,277.50 annually. ($32.45/hr). JOB OBJECTIVES: To support individuals with a primary diagnosis of development disability or related condition. This position provides service coordination and works collaboratively with individuals, families, provider agencies, and community partners to ensure each individual has the supports and services needed for a healthy, safe, and fulfilling life.

ESSENTIAL DUTIES AND RESPONSIBILITIES: The duties listed are intended only as illustrations of the various types of work that may be performed. Provide person-centered case management services that assist individuals who receive services in locating and gaining access to needed waiver and other state plan services while meeting the person’s goals related to medical, social, housing, employment, and various other goal areas.Become certified in and conduct MnChoices assessments.With the individual’s involvement, develop and implement a person-centered Coordinated Services and Support Plan (CSSP) that is based on the MnChoices assessment results and a Consumer Directed Community Supports Plan when appropriate. Evaluate, facilitate and monitor services and individual progress in relationship to established goals and objectives. Works with the person to adjust service plans as necessary.Plan and arrange for services and prepare appropriate service agreements.Act as a liaison with community and referral services.Prepare and maintain case records which meet federal, state, and local statutes/rules. Under the direction of supervisor, seek court intervention when needed to protect the health/safety of vulnerable children or adults: prepare reports needed for court, testify in court as may be necessary, implement court orders.Participate in team meetings, agency all staff meetings, county meetings and trainings.Perform other duties as assigned or apparent. Complies with all rules and policies in order to maintain a safe work environment.Reliable, predictable attendance and punctuality. Access to Not Public DataThe incumbent may encounter not public data in the course of these duties. Any access to not public data should be strictly limited to accessing the data that are necessary to fulfill the employment responsibility. While data are being accessed, incumbent should take reasonable measures to ensure the not public data are not accessed by individuals without a work reason. Once the work reason to access the data is reasonably finished, incumbent must properly store the not public data according to the provisions Ch. 13.If a new work assignment requires access to not public data, the incumbent is permitted to access not public data for the work assignment purposes only. Any access to not public data must be strictly limited to the data necessary to complete the work assignment and after the assignment is completed, the employee’s work assignment no longer requires access.

WORK ENVIRONMENT Public building and working with the public both in the office and out in the community.May involve occasionally interacting with hostile and unstable clients.Exposure to the elements when out in the field, including icy roadways, unpaved roads, untrained or aggressive pets, unsanitary/unsafe living conditions, heat, rain, cold, and snow. ($32.45/hr). *Receive anniversary step increases for earning potential of $81,256.50 ($41.67/hr).
