Join our team as an admissions coordinator!

Working as an admissions coordinator, you will be responsible for:

• Coordinating admissions with hospital discharge planners and other referral sources
• Assessing, coordinating and ensuring a smooth admission transition
• Confirming medical and financial information
• Developing and maintaining excellent networking skills and relationship building with referral sources
• Ensuring patient continuity of care

Required Qualifications

• ability to multitask, manage multiple priorities, and work in a fast-paced environment
• detailed oriented with strong organizational and communication skills
• previous healthcare experience
• knowledge of various computer applications - experience with EPIC, Care Anywhere, and Point Click Care (PCC) is preferred
