Job Description
Current license from the Pennsylvania State Board of Pharmacy
Experience : Completion of ASHP accredited PGY1 pharmacy residency in acute care hospital pharmacy or Board Certification or 3 years advanced pharmacy practice.
Other Skills : Good written, verbal, and presentation skills are essential. Must be able to analyze and interpret complicated information to determine a course of drug therapy. Must possess basic computer skills, time management skills, and the ability effectively communicate with others to solve complex problems.
- PURPOSE OF JOB :
- ESSENTIAL FUNCTIONS : (Maximum of 6)
- Review medication orders for accuracy and optimize care based on appropriate indication, dosing, route, toxicity, drug interactions, drug allergies, cost-effectiveness, and evidence-based pharmacotherapy.
- Participate in direct patient care activities including, but not limited to, patient counseling/ education on medication use, medication reconciliation, and participation in emergency code response as needed.
- Provide drug information to physicians and other health care professionals and assist in medication-related questions.
- Participate in the quality improvement and medication use review activities of the Pharmacy department on hospital committees. Collects and analyzes data, conducts quality monitoring and inspections. Maintains appropriate logs, records, and other documentation as assigned.
- Develop and oversee the competency of pharmacists and student pharmacists in specialized area(s).
- Exercise applicable legal and professional controls on controlled and non-controlled substances maintained and dispensed .
- Maintain competency in all administrative and miscellaneous requirements of the department
- QUALIFICATIONS :
