POSITION SUMMARY:
Reports directly to the Assistant Clinical Director and Clinical Director. Responsible for case management at a behavior health residential and outpatient level treatment facility.
DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:
Maintain client case load
Facilitate completion of FMLA documentation as needed
Complete client care portion of intake process
Facilitate tour of residential grounds, including locations of posted schedules for appointments and UA’s
Complete exit strategy plans
Provide clients with the most recent schedule to keep on their persons
Perform client care management tasks necessary to meet clients’ needs, ensuring the highest level of quality and customer service
Assist clients with the achievement of treatment goals
Organize and facilitate psych0-educational groups to clients
Maintain accurate and complete records of correspondence and services rendered to client
Communication and coordination of client care regarding outside entities
Liaisons and communicates to the family members, marketers and referent sources as needed
Assists with client money passes / visit passes
Attends clients staffing meetings weekly
Other duties as assigned (not listed above)

None
WORK ENVIRONMENT:
Behavioral Health Residential and Outpatient level treatment facility for substance abuse and co-occurring disorders.
PHYSICAL DEMANDS:
Must have reasonable ability to stand, walk, sit, handle, feel, reach, climb or balance, stoop, kneel, crouch or crawl, talk, hear, taste, and smell. Must be reasonably capable of lifting 20 pounds.
WORK HOURS:
Position can be Part Time / Full Time, Non-Exempt.
May be required to work holidays, weekends and any shift within a 24-hour day.
Variable hour shifts (typically 12 hour).

Requirements
QUALIFICATIONS FOR POSITION:
• Bachelor level or higher in related field (behavioral health)
• Two or more years of full-time direct work experience in a therapeutic work setting
• Must maintain the highest level of confidentiality
• Effective scheduling and coordination skills
• Must be open-minded, flexible and willing to take direction
• Must possess good verbal and written communication skills
• Must work well in a crisis setting and be willing to learn new crisis intervention techniques
