What are the responsibilities and job description for the Long Term Substitute School Nurse position at DR. Philip O. Coakley Middle School?
• Responsible for the planning, implementation and follow-up of all state mandated School Health Programs, including the following:
• Routine and athletic physical examinations for Gr. K, I, 4, 7 and I0;
• Vision and hearing screening in all grades unless a student receives a waiver from the Massachusetts Department of Health;
• Postural screening in Grades 5-9;
• Height and weight measurements in all grades;
• Implements Medication Policy as ·determined by the medical/ nursing staff.
• Maintains, reviews and updates accurate statistics on all programs; including communicable disease and immunization recording as
mandated by the Massachusetts Department of Health;
• Maintains tuberculin screening records of all new pupils as per School Policy; and
• Maintains and updates cumulative student health records.
