What are the responsibilities and job description for the Service Director/Case Manger for HCS/TxHML Programs position at Draco Services, Inc?
Draco Services is in need of a HCS/TXHML Service Director to serve our community of individuals with intellectual and developmental disabilities in and around the Austin area.
Job Responsibilities: Being responsible for a case load of 50-60 consumers candidate will be responsible for measurable progress of his/her caseload of assigned residential consumers. Candidate will also be required to write and coordinate Implementation Plans for each consumer in their caseload, as well as maintain high quality services, meeting documentation requirements and company financial goals. The candidate will manage all requirements in regards to Host Home guidelines and Community First Choice guidelines.
Minimum Requirements: Case Management Experience. High school graduation or GED. Demonstrated team building skills to effectively work with others resulting in meeting goals; must possess and maintain a valid TX driver’s license; have and maintain a driving record that is insurable under company's auto insurance; ability to pass state registries and criminal background check at hire and annually thereafter. Two years experience working in Home and Community Based Services (HCS) preferred. Bi-lingual Spanish/English preferred, but not required.
This position requires 75% of travel. The right candidate must work independently as he/she assesses the needs of the persons receiving services through completion of periodic assessments & documentation, such as the following:
- IPC
- IDRC
- ICAP Booklet and electronic
- Implementation Plan
- Complete needed IPC revisions
- Onsite Inspection
- Individual Emergency Evacuation Plan
- Financial/Room & Board Disclosure
- 6 month reviews of IPC plan year
- Oversee delivery of all HCS program services in adherence with regulatory principles, and state and federal laws (including HIPPAA Privacy Laws) concerning confidentiality. Monitor to ensure all medical services are provided in accordance with the individual's plan of care.
