Duties
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop, implement, and monitor individualized care plans in collaboration with clients and their families.
- Provide crisis intervention and support during critical situations.
- Facilitate access to services such as behavioral health support.
- Mentor clients to promote self-sufficiency and empowerment through motivational interviewing techniques.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.

- A background in social work or a related field is preferred.
- Experience in behavioral health and crisis intervention is highly desirable.
