Front Range Staffing is seeking an experienced Dual Diagnosis Case Manager for a 3-month contract in Colorado Springs. This position is full-time, day shift, Monday through Friday. This position is urgently hiring, must be available to start immediately.
Pay starts at : $24 to $25 per hour, depending on experience.
An ideal candidate for this company would possess the following skills and experience :
- Experience working with individuals with co-occurring disorders ensuring compliance with program requirements and legal obligations.
- Conduct risk assessment interviews to evaluate clients' needs and determine appropriate interventions.
- Develop and maintain individualized case plans, tracking progress and documenting concerns in records.
- Plan, coordinate, and facilitate group sessions aligned with program curriculum and contractual guidelines.
- Communicate regularly with external parties to provide client progress updates.
- Complete intake procedures for new clients, informing them of program rules, policies, and expectations.
- Prepare and submit various reports, including termination summaries, program evaluations, and client budget plans.
- Identify and refer clients to appropriate services, such as mental health treatment, substance abuse programs, and housing resources.
- Lead psychoeducational classes and conduct cognitive behavioral interventions to support client rehabilitation.
- Assist clients with discharge planning, collaborating with referring agencies to ensure a smooth transition.
- Work closely with service providers, government agencies, and community resources to secure necessary support for clients.
- Strong case management, advocacy, and crisis intervention skills.
- Excellent organizational, communication, and documentation skills.
- Ability to collaborate with legal and social service agencies.
- Proficiency in electronic health records and case documentation.
- Bachelor’s in Social Work, Psychology, Criminal Justice, Counseling, or a related field.
