POSITION OVERVIEW
As Market Medical Director at Duo Health, you'll transform healthcare delivery as a key leader in one of our core markets, combining hands-on clinical care with strategic and operational leadership. You'll maintain a focused panel of high-risk patients while building and leading our clinical teams of Nurse Practitioners and PAs, pioneering an innovative care model that meets patients where they are – in their homes, dialysis facilities, and hospitals. Partnering with the Market General Manager, you'll drive patient engagement, ensure clinical excellence, and optimize outcomes. As a visionary physician who values outcomes over volume, you'll develop distributed teams and create a differentiated care model that puts patients first through comprehensive, coordinated care.
SCOPE OF RESPONSIBILITES
- Design and implement market-specific protocols for transitional care management, ensuring seamless coordination across hospital systems, dialysis centers, and home care settings while maintaining strong relationships with key facility partners.
- Develop and execute physician engagement strategies, including creating collaborative care protocols with nephrologists, primary care physicians, and other specialists to enhance patient care coordination.
- Establish market-specific quality assurance programs that align with Duo Health's national framework while accounting for unique market dynamics and population health needs.
- Create and oversee mentorship programs specifically designed for distributed care teams, including virtual coaching sessions, ride-along programs, and peer learning opportunities that enhance clinical capabilities in the home care setting.
- Implement innovative approaches to address social determinants of health, including partnerships with community organizations, transportation services, and local support systems to enhance care accessibility.
- Lead the evaluation and deployment of new clinical technologies within the market, ensuring successful adoption by care teams and meaningful integration into daily workflows to improve care delivery efficiency.
- Develop market-specific risk stratification models that account for local population health trends, social determinants, and available resources to optimize care team deployment and patient outcomes.
- Design and implement programs to improve care transitions and reduce readmissions, including establishing relationships with local emergency departments, post-acute care facilities, and community providers.
- Create and maintain clinical scorecards that track key performance indicators, including quality metrics, patient satisfaction scores, and medical cost management targets, with regular reporting to national leadership.
