We are excited to offer the opportunity to work as a Public Health Nurse in our Family Health Services department. We have current availability at our Southeast Public Health Center, located in Westmont, Il on a full-time basis. This is a 37.5 hour per week position. The hours are generally Monday through Friday from 8:00 AM until 4:30 PM. One to two days per week could be until 7:00 pm. Occasional evenings and weekends.
- Provides direct public health nursing, family case management, WIC nutrition counseling, and breastfeeding support to clients
- Administers nursing services to pregnant, infant, and children in a clinic and community setting
- Provides home visits, physical assessments, and referrals for at risk infants, and children identified through the adverse pregnancy outcomes reporting system
- Applies fluoride varnish for infants and children in a clinical setting
- Enrolls eligible clients and provides coordination of care and support to program participants
- Educates clients, both individually and in small groups
- Develops individual care plans for clients enrolled in the programs
- Provides appropriate referral information to clients
- Administers vaccines, perinatal depression screenings, and ages and stages questionnaires
- Counsels individuals and families with their health needs and concerns
- Completes required documentation in the electronic medical record
- Participates in emergency response activities as assigned
Salary : $60,300 - $80,400
