GENERAL DESCRIPTION OF DUTIES
The primary purpose of this position is to assist clients and their family members as well as other health professionals and staff within the health department or community to determine a client’s nutrition needs and ensure they receive appropriate clinical nutrition services. Work includes providing clinical nutrition services and medical nutrition therapy to clients with comprehensive nutrition needs through in-depth nutrition assessment and counseling to clients requiring therapeutic intervention of moderate to high-risk nature in addition to providing basic nutrition services to clients eligible for Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).
SPECIFIC DUTIES AND RESPONSIBILITIES
EXAMPLES OF ESSENTIAL FUNCTIONS
70% WIC Nutrition Services
· Use nutrition assessment information to determine eligibility for WIC.
· Provide individual and small group nutrition counseling on food groups and child development nutrition needs.
· Analyze clinical and nutritional data to prescribe appropriate supplemental foods based on the clients’ nutritional needs and food preferences.
· Interprets laboratory information.
15% WIC Client Services
· Use nutrition assessment information to determine eligibility for WIC.
· Provide individual and small group nutrition counseling on food groups and child development nutrition needs.
· Analyze clinical and nutritional data in order to prescribe appropriate supplemental foods based on the client’s nutritional needs and food preferences.
10% Medical Nutrition Therapy
· Provide comprehensive and complex medical nutrition therapy to pregnant women, adults, children, and infants who have one or more nutritionally related chronic or acute disease conditions, such as but not limited to: metabolic disorders (diabetes, polycystic ovary syndrome, obesity); acute and chronic infections such as HIV; growth disorders (e.g. failure to thrive/underweight); genetic abnormalities; developmental, sensory, and motor disabilities; pulmonary disease; conditions that impact the length or outcome of gestation; and immunological disorders such as food allergies.
· Conduct a comprehensive nutrition assessment on each patient using anthropometric and biochemical measurements, clinical, dietary, and socio-economic information to write a nutrition care plan.
· Provide in-depth individual counseling concerning therapeutic diets, developmental processes, programmatic parameters, and nutrition principles.
· Determine an appropriate food prescription.
· Develop a realistic plan of care with the patient and other health care providers.
· Counsel and assist clients with behavior changes to meet medical goals.
· Discuss treatment plans and dietary modifications with the client and with other members of the health team.
· Provide case management as necessary and identify community resources.
· Assimilate and integrate psychosocial information, cultural issues, and literacy levels into counseling sessions and overall plan of care.
· Research nutritionally related medical problems using current nutrition and medical texts and literature accessed through searching medical journal databases.
· Document all aspects of nutritional care in the electronic medical record according to the agency’s policies and procedures.
· Assist with the Diabetes Self-Management Education Program.
· Develop nutrition educational resources such as presentations and handouts.
· Review nutrition education resources for appropriateness with targeted populations.
· Participate in Quality Assurance.
MARGINAL FUNCTIONS
While the following tests are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.
5% Other duties
· Attend professional workshops and conferences for current nutritional trends, problems, and state-of-the-art delivery methods to improve the quality of services.
· Assure all clinical nutrition services are provided in compliance with HIPAA.
· Provide input into marketing of nutritional services.
· Develop nutrition educational resources for appropriateness with targeted populations through use of presentations and handouts.
· Participate in Quality Assurance.
· Participate in shelter duty and other activities relating to the Department’s emergency response and other disaster relief efforts.
· Serve as a team member during outreach projects such as flu vaccine season and disease outbreaks.
· Perform any other duties and responsibilities as directed by the supervisor.
MINIMUM TRAINING AND EXPERIENCE
· Registered Dietitian with the Commission on Dietetics Registration; or
· Master’s degree in Dietetics, Public Health Nutrition, or Nutrition; or
· Graduation from a Commission on Accreditation for Dietetics Education-approved Didactic Program in Dietetics and two years of nutrition experience; or
· Dietetic Technician, Registered with the Commission on Dietetics Registration with a Bachelor’s degree in any subject area from an accredited four-year college or university and two years of nutrition experience; or
· Bachelors of Science degree in Dietetics, Public Health Nutrition or Community Nutrition from an accredited four-year college or university and two years nutrition experience.
