Key Responsibilities
- Collaborate with the Community Nutrition Educator to coordinate nutrition health education sessions aimed at enhancing health outcomes for Spanish-speaking HIV clients.
- Maintain accurate client records and systematically document participation in program activities focused on nutrition health education.
- Initiate and follow up on referrals for essential community services, such as food programs and entitlement resources.
- Conduct off-site intakes and assist in scheduling appointments for screenings with the Community Nutrition Educator.
- Engage in program outreach efforts and work closely with medical providers and Registered Dietitians to ensure clients are retained in care.
- Maintain ongoing communication with Spanish-speaking program clients, documenting their progress towards achieving individualized nutritional goals.
- Responsible for the distribution of weekly pantry bags to clients.
- Bilingual proficiency in English and Spanish, with strong communication skills in both oral and written forms.
- Familiarity with community services available to Persons Living with HIV/AIDS in Nassau County.
- Proficient in computer applications, including Microsoft Office and data tracking systems.
- Strong interpersonal skills to work collaboratively with clients and other service providers.
- Associate’s degree in health or human services is required; a Bachelor’s degree is preferred.
- Minimum of 1 year of experience in the HIV/AIDS field or with other chronic illnesses.
