EAC Network, a not-for-profit social service agency that empowers, assists, and cares for 62,888 people in need through 100 programs across Long Island and NYC, seeks a Peer Mentor to work part time for the Brooklyn Criminal Treatment Courts Case Management & Recovery Support Services program.

The Peer Mentor position is a non-exempt position paying $24 per hour, Monday and Tuesday 9AM- 5PM. The office is located in Brookyn, NY. The role reports to the Program Supervisor.

 

Primary Purpose of Job:     

The Peer Mentor’s primary job responsibilities are to provide peer support, advocacy, and linkage services to individuals enrolled in the program. The successful candidate must be a self-starter and be willing to use their lived experience to help others who are seeking recovery from substance use issues. The Peer Mentor will work as a fully integrated team member providing culturally responsive peer recovery support services to individuals in the Brooklyn Criminal Treatment Courts Case Management & Recovery Support Services program.  

 

Principal Duties & Responsibilities:

  1. The Peer Mentor will provide support for individuals enrolled in Criminal Treatment Courts Case Management & Recovery Support Services program that are having difficulty connecting to services.
  2. Provide a peer-to-peer relationship to support individuals in recovery and family members with sustaining recovery and overall wellness so they can achieve their goals with in-person visits at the office, or in the community and/or telehealth services.
  3. Assist program staff with implementation of treatment goals to help individuals achieve their goals.
  4. Willing to escort participants to treatment providers and appointments as needed.
  5. Utilize program assessment tools to monitor the client's mental health/substance use and care needs.
  6. Provide information and referrals to individuals and families for social and recovery support services, counseling, and linkages to prevention and treatment services.
  7. Provide harm reduction education, such as Narcan kits, Fentanyl/Xylazine strips, and other information to enrolled individuals.
  8. Participate in supervision and team meetings; participate in agency trainings and workshops, and other trainings.
  9. Prepare and maintain client case records and statistics as required by the standards set by EAC grant guidelines.
  10. Document in the NYS Unified Court system program all services provided to participants and maintain all aspects of documentation in accordance with regulatory requirements.
  11. Be willing to facilitate peer run groups.

 

Knowledge, Skills, & Abilities Required:

  1. Minimum of High School Diploma or educational equivalent required
  2. NYS Certified Recovery Peer Advocate (CRPA) or Peer credential preferred
  3. Lived experience in substance use and have knowledge of the criminal justice system.
  4. Good written and verbal communication.
  5. Must possess computer skills and the ability to navigate the electronic health record (EHR).
  6. Knowledge of substance use disorder recovery process.
  7. Understand the therapeutic benefit of Medication Assisted Treatment (MAT)
  8. Able to work with diverse populations (ethnicity, sexual orientation, socioeconomic status, etc.).
  9. Demonstrate commitment to providing quality services and adhering to outcome driven performance measures.
