Holmen Campus Nurse
Eagle Crest Communities is seeking a Nurse to join the
Holmen Campus serving the residents of our 12-unit memory care home and 16
assisted living apartment community.
The successful candidate will partner with the campus
manager, provide direction and leadership to our team of dedicated and hardworking
Resident Care staff.
What You’ll Do
- Communicate with residents, families, and
providers, in the coordination of care
- Actively maintain Individual Service Plans and
Resident Risk Agreements
- Responsible for oversite and provision of
medication management and treatment services, physical and behavior assessments
or the residents
- Duties will be performed in compliance with CBRF
regulations (DHS 83) and RCAC regulations (DHS 89) and policies and best
practices established by Bethany Lutheran Homes, Inc.
What You’ll Need
- Must be licensed to work in the State of
Wisconsin as a Licensed Practical Nurse or Registered Nurse.
- 1 year in a position that provides oversite
regarding medication administration and clinical duties for direct care staff.
- Experience working with geriatric population
and/or memory care is beneficial, but not required—we will mentor those who fit
with our culture and core values!
- Drivers require a valid driver license and must
have and maintain a safe driving record
