Adult Day Habilitation Program Manager

Summary : Responsible for the supervision and safety of consumers. Responsible for overseeing and coordinating implementation of treatment / service plans, including behavior management and contingency programs. Provides instruction in Basic Living Skills and Family Support and Education as outlined in the consumer's treatment plan and under the direction of a therapist.

Consumer Care : Supervises consumer activities, including completion of daily chores, outings, training, grooming, bathing, toileting, meal-time activities. Ensures consumer safety. Follows a schedule of activities. Monitors consumer physical and mental health, ensuring that consumer needs are met and maintained. Assists consumers as needed, to include lifting and other physical intervention as needed. May administer consumer medication as prescribed by a physician and accompany consumers to medical appointments. Reports and documents incidents and / to unusual problems to supervisory or on-call staff. Models appropriate social skills. Ensures consumer rights are observed and protected. Ensures consumer clothing needs are met and that consumer is provided a suitable living environment. Completes program chores as assigned; i.e. sweeping, dusting, cooking, shopping, washing and drying consumer clothing, changing linen. Follows menus approved by dietician. Assists with menu planning (when making substitutions to menus ensure approval by dietician), following menus, and preparation of meals. Ensures that meals are served family style and that appropriate table manners are modeled by staff during meal time. Ensures compliance with special diets. May schedule consumer appointments for needed services (medical, dental, etc.). Responsible for reporting refusal of medications, medication side effects, or any problems with medication administration to nursing staff.

Program Services : Assists in implementation of the consumer's treatment, habilitation, and / or service plans. Assists with implementation of the program's contingency program (i.e., level system, token economy, etc.). Implements behavior management procedures as assigned and / or as indicated by a contingency program or the individual's behavior plan. Contacts consumer relatives or other significant others as appropriate. Completes fire drills, severe weather drills, and nightly bed checks. Reviews consumer records to ensure familiarity with each consumer's social and medical history.

This is a full-time position with the hours being 7am - 3 : 30 M-F.

Skills & Requirements

• Bachelor's degree, in a human or social services fields of study.
• Valid driver's license for the state of Alabama and safe driving record.
• Must meet conditional employment requirements.
• Must be able to enter data into a computer.
• Able to read, write and speak in English.
• Must be able to use good judgment in dealing with work related issues.
• Dependability is a key requirement for the position.
• Must be able to effectively communicate work related incidents to the appropriate persons.
