What are the responsibilities and job description for the Case Manager/Housing Advocate position at East Bay Community Action Program?
East Bay Community Action Program is hiring for a full-time, Housing Advocate/Case Manager. The Housing Advocate/Case Manager provides enrollment and case management for EBCAPs Pay for Success program. This position will perform activities related to enrollment, case management and referrals.

Key Responsibilities:
- Provide case management services that link clients to services that will lead to housing stabilization. This includes interviews and assessments of referred clients for placement in the Pay for Success program.
- Complete home visits to assess clients current living situation and develop goals for continued home stabilization.
- Participate in regularly scheduled supervision with Manager of Housing Support Services and/or Senior Director Workforce Development & Social Services.
- Be proficient in HMIS data entry and attend all required Pay for Success/Housing meetings.
Qualifications:
- Bachelors degree in human services or related field preferred
- 1 year experience working with housing case management.
- Communicate and coordinate with other EBCAP programs, to accept referrals for EBCAP clients who have the need for other services.
- Be able to complete comprehensive/holistic assessment and implement/follow up with appropriate referrals/linkages including selected resources.
- Establish trusting/non-judgmental relationships and set appropriate boundaries.
