Job Description Summary
The PrEP Navigator works with the Primary Care Prep Expansion team and Prep Coordinator to provide services for patients who do not have HIV and have self-identified or have been referred as high-risk for HIV infection. The PrEP Navigator has knowledge on HIV/AIDS/STI prevention, transmission, and epidemiology. The PrEP Navigator provides guidance, counseling and advocacy to clients in need of assistance accessing Pre-Exposure Prophylaxis (PrEP), Non-Occupational Post Exposure Prophylaxis (nPEP), Sexually Transmitted Infections (STI) testing and comprehensive medical care. Together the PrEP team schedules intakes and follow up appointments, coordinates treatments and refills with the provider, and provides sexual health education. The PrEP Navigator will also provide Counseling and Testing Services to patients on PrEP.

Duties and Responsibilities:
- Collects information during risk assessments/sexual histories and refers patients to appropriate services.
- Provides health education, creates risk reduction plan with patient, and provides medication adherence support.
- Connects patients to benefits programs such as Mass Health/Health Connector, PrEP Drug Assistance Program (PrEPDAP), Copay Cards, and other programs.
- Maintains concise and accurate documentation according to departmental standards including but not limited to chart notes in patient electronic medical record, MA DPH forms, internal database, and other forms required by funders.
- Facilitates timely scheduling to ensure proper testing, treatment, follow-up, and refills.
- Provides HIV/STI counseling & testing: is able to provide client-centered HIV/STI counseling services and has phlebotomy certification/experience or a willingness to be trained in phlebotomy.
- Tracks and documents patient follow-ups and barriers to follow up.
- Serves as a key contact to Primary Care and other EBNHC clinical departments providing follow up/support referrals and assistance with PrEP services as outlined above.
- Will work closely with health educators and other patient navigators to ensure aligned workflows, processes and documentation.
- Provides training on PreP services at EBNHC to new staff and clinicians.
- Assists with reporting, CQM and compliance requirements.
- Collaborates with the rest of the Project SHINE team in outreach efforts: such as testing day and HIV/AIDS awareness days, social media campaigns for PrEP education/outreach, and PrEP/nPEP education of EBNHC interdepartmental providers.
- Collaborates with MA DPH Field Epidemiologist when referrals are appropriate.
- Stays up to date with PrEP developments as new medications and new methods are approved.
Requirements:
- Bachelor in Science or Arts preferred or equivalent experience in related field
- Fluent in Spanish or Portuguese preferred
- Community engagement; health education and promotion
- Eliminating access to care and addressing social determinants of health
- Experience working with diverse and/or special populations, including but not limited to Latinx community, LGBTQIA , non-US born persons, persons who inject drugs.
- Working in community health setting
- Utilization of social media; and workshop facilitation
- Must have basic computer, communication, and writing skills
