What are the responsibilities and job description for the Community Care Worker position at East Central Ohio Educational Service Center?
The Community Health Worker (CHW) is a new position designed to handle community care coordination in Belmont County. This position will be responsible for helping clients and their families navigate and access community services, other resources, and adopt healthy behaviors. Through home visiting, the CHW will work with clients in overcoming barriers to health, social services, education and employment. They will also provide social support, informal counseling and advocate for individual and/or community health needs.
Essential Duties & Responsibilities:
- Uses the Pathways protocols for care coordination activities at all times in managing assigned caseload of clients
- Establishes trusting relationships with clients and their families while providing general support and encouragement
- Interviews clients with problems such as personal and family adjustments, health, finances, employment, food, clothing, housing, utilities, and physical/mental impairments; completes appropriate checklist and enters information from home visits into database no later than 48 hours after client contact
- Follows-up with clients via phone calls, home visits and visits to other settings where clients can be found
- Provides referrals for services to community agencies as appropriate, helps clients connect with transportation resources and gives appointment reminders
- Advocates for clients and acts as a liaison between the clients/family and community service agencies (i.e., schools, hospitals, support groups, etc.
- Counsels clients individually, in family, or other small groups regarding plans for meeting needs and aids clients to mobilize inner capacities and environmental resources to improve social functioning
- Helps clients and family through individual or group conferences to understand, accept, and follow medical recommendations
- Determines client’s eligibility for services such as financial assistance, insurance and other programs in place to assist individuals and refers clients to community resources and other agencies to meet identified needs
- Attends CHW meetings, trainings and other meetings as required
Knowledge/Skills:
- Knowledgeable about community resources appropriate to the needs of clients/families
- Effective time management practices (able to organize time, prioritize projects efficiently and effectively)
- De-escalation skills and problem solving skills
- Effective in managing change and conflict
- Working knowledge of computer software
- Commitment to agency mission statement – able to develop trust and effective working relationships with staff members, individuals, families and external/internal professionals
- Strong information gathering/interviewing techniques
Education & Experience
- High school diploma or general education degree (GED)
- Formal training and/or related coursework from a college or other education institution preferred
Certificate/Licensure/Registration:
- Must successfully complete the CHW training within 6 months of hire date and be certified through the Board of Nursing
Additional Information:
- Successful completion of BCI/FBI background check; enrollment in Rapback required
- Valid driver’s license
- 260 Day contract
- Starting salary $37,500 – negotiable based on experience and background
- 2 weeks vacation all ECOESC holidays
