What are the responsibilities and job description for the Community Health Worker position at My Community Health Center?
PURPOSE OF POSITION
The Community Health Worker (CHW) is an integral member of the THRIVE outreach team to reduce Infant Mortality in Stark County. The Community Health Worker (CHW) will be responsible for helping pregnant women navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Care Management Coordinator through an integrated approach to care management. As a priority, activity will promote, maintain, and improve the health of patients and their family. Provide social support and informal counseling, advocate for individuals and community health needs, community outreach, such as home visits and health screenings will be required.
RESPONSIBILITIES & EXPECTATIONS
- Provide assistance to clients in the OB Clinic centering program to address social determinants.
- Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
- Providing ongoing follow-up and goal setting with patients through home visits.
- Conduct intake interviews and complete paperwork for entry into the HUB.
- Enroll and/or refer patients into community programs assisting with applications/registrations where necessary.
- Ensure that patients attend appointments.
- Utilize the THRIVE’s chosen curriculum “Partners for a Healthy Baby Training” to provide education for prenatal, pregnancy and postpartum.
- Help patients connect with transportation resources and give appointment reminders in special circumstances. (Transporting patients is strictly prohibited.)
- Exhibit excellent working relations with patients, visitors and clinic staff, effectively communicating THRIVE and the MCHC OB Clinic.
- Work closely with providers to ensure coordinated care.
- Be knowledgeable about community resources appropriate to needs of patients/families.
- Be responsible for providing consistent communication to the CCS Supervisor to evaluate patient/family status and ensure data is entered properly into the HUB
- Act as a patient advocate and liaison between the patient/family and community service agencies (i.e. schools, JFS, hospitals, support groups, etc.).
- Attend regular staff meetings, trainings and other meetings as requested.
- Understand and effectively use EMR and other computer applications such as Outlook, Word and Excel
- Create and use spreadsheets to track centering patients and outcomes
- Be proactive in learning and utilizing the Pathways HUB
- Actively engage in teamwork
- Support the mission of MCHC
- Effectively use Tools and Tones for Positive Communication
- Other duties as assigned
JOB QUALIFICATIONS
- Must be over 18 years of age
- High School Diploma or General Education Diploma (GED), is required
- Must be able to travel with use of a personal vehicle.
- Certification from Ohio State Board of Nursing for Community Health Worker (CHW)
- Must live, work and receive services in the Stark County community that you serve
- Ability to operate a PC/laptop/tablet to enter data components.
- Preferred: Two (2) years of experience in either a volunteer or paid position, working in community settings with at risk populations and providing coordination of services
- Strong interpersonal skills and teamwork mentality
- Adjust easily to changes in duties, schedules and clinical staff
- Function independently, have flexibility to work with all levels of personnel; clinical, professional and clerical
- Flexibility of moving throughout front office to cover for vacations and/or call offs to meet the needs of any clinic on any day
- Professional conduct in a clinical office setting
- Problem solving skills

Pay: From $16.00 per hour
