What are the responsibilities and job description for the Patient Advocate position at East Neck Nursing & Rehabilitation Center?
East Neck Nursing & Rehabilitation is looking for a Patient Advocate to join our dynamic team of professionals! This is an exciting opportunity to work with all levels of personnel at our facility as well as our residents and their families. This position is full-time, and the hourly rate is $21.
The primary purpose of this position is to meet with residents daily to ensure that any issues they may have are addressed. You would be responsible for greeting new residents and making sure that their needs and questions are being attended to.
Qualifications:
- Must be able to move intermittently throughout the day.
- Must be able to read and write the English written language in an understandable manner.
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
- Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
- Must be in good general health and demonstrate emotional stability. Must be able to stand, lift, push, and pull at least 30 pounds.
- Will address concerns addressed by residents and their families.
- Will greet new admissions within 72 hours of their admission.
- Will contact interdisciplinary team members to address any immediate concerns and as necessary will contact administrative team to ensure appropriate resolution of concerns.
- Will complete the log of all new admissions, noting dates and times of contacts, concerns identified and how they were addressed.
- Will participate in morning report/interdisciplinary QA meetings as appropriate to communicate issues of concern to the team and make recommendations on how to improve resident and family satisfaction.
- Will report all findings daily at morning report. All immediate issues will be addressed when they occur. Will make recommendations on how to provide recognition to staff members providing outstanding customer service and will recommend to administrative staff what additional training staff may benefit from to better provide quality customer service.
- May serve as a translator as needed to facilitate effective communication with resident.
- Will conduct daily rounds throughout the facility, specifically all resident units to ensure cleanliness, clutter-free, free of unpleasant odors, identify any safety concerns, or any other environmental issues that may require follow-up. Will submit daily report of rounds to department head.
- Will participate in meetings as assigned for Compliance, Safety, Motivation and Moral, and WIRD.
- Special projects as assigned.
- All other duties as assigned.
