The East River Family Strengthening Collaborative (ERFSC) is a non-profit organization whose mission is to empower families, youth, seniors, and communities to become more self-sufficient through integrated and collaborative community-based services utilizing evidence-based practices that are family focused and person- centered.
Job Summary
Service Coordinator will coordinate and implement the Individual Socialization, Counseling, and Information and Referral Assistance services for participants for community residents aged 60 . The Service Coordinator will also travel to the various community dining sites and senior locations in the ward and provide informational sessions to participants. The Service Coordinator is also responsible for providing these services to isolated homebound seniors, which may include visits to the home.
Knowledge, Skills, and Abilities
Knowledge of the needs and problems of the aging; Knowledge of senior services, socialization, community partnerships and outreach; Able to learn and work professionally with others. Knowledge of customer service; Knowledge of issues facing seniors in the community; Knowledge of and the ability to follow confidentiality protocols; Able to proactively solve problems; Able to work with community residents who may be need additional help with navigating programmatic processes; Able to remain patient; Able to exercise good judgment in evaluating situations and making decisions; Able to define and evaluate problems and determine or suggest solutions; Able to organize and prioritize multiple tasks; Able to follow East River policies and procedures.
Education and Experience
Master’s degree in social work required. Two to four years of experience in social work and/or case management preferred. Knowledge of information and referral best practices is required. Prior experience working in East River senior services preferred.
Licenses and Certifications Valid Driver’s License require; LICSW preferred but will consider MSW with at least two years of experience or MSW with LGSW

Working Conditions Indoor office environment 85% of the time; in the community and other agencies and organizations 15% of the time.
