Location: Knoxville, Tn.
Employment Status: Full-time
Hourly rate: $24.50
Duties/Responsibilities:
- Conduct in-home assessments for older clients and persons with disabilities for possible enrollment in the CHOICES Home and Community Based Program.
- Service Area is Knox and surrounding counties.
- Completion of the TennCare Pre-Admission Evaluation and TennCare Financial Application while in the home.
- Gathering supporting documentation to support the TennCare Applications.

Qualifications/Requirements:
- Graduation from an accredited four year college with a Bachelor's degree in Social Work, Nursing, Education, or closely related fields (Psychology or Sociology); or
- An individual who is a Licensed Nurse (LPN), Registered Nurse (RN), Licensed Social Worker, Licensed Physician, Nurse Practitioner, or Physician Assistant;
- Good organizational, interpersonal skills and decision making skills; or
- Any equivalent combination of education and/or experience will be considered
- Valid TN driver license.
