What are the responsibilities and job description for the Nurse Navigator position at East Texas Kidney Specialists?
We are looking to add a motivated full-time associate to our growing practice at East Texas Kidney Specialists in Longview, TX. We are a fast-paced, 5 physician nephrology practice providing care to patients coming from all across East Texas. As a nurse navigator, you play a vital role in supporting our patients and healthcare team.
Essential Functions: Demonstrate clinical competence in the direct care of patients and the following patient needs to assure high standards of care. Functions in an accurate and efficient manner.
· CKD Education
· Medical Nutrition Therapy Referral
· Vascular Access Referral
· Supportive Care Referral if appropriate
· Advance Care Planning if appropriate
· Dialysis Modality Preference Discussions
· Follow-Up Care Intervals for this group of patients
· Pre-emptive Transplant Referrals when appropriate
· Discussion of In-Center Initiation of Dialysis when time appropriate
· Transitional Care Management for hospital discharges
Areas of importance for focus of this position:
· Reduction of initiation of dialysis with catheters
· Increase the number of in-center initiation of dialysis
· Initiating dialysis with a functioning AV fistula
· Increase number of pre-emptive transplant referrals (eGFR<20)
· Discuss home dialysis modalities with all candidates
· Refer patients for evaluation for fistula and vein mapping evaluation
Performs necessary duties required for the efficient operation of the office:
· Participates in reviewing laboratory tests and assists with patient phone calls as necessary.
· Handles telephone calls from patients/family members in a courteous, informative manner. Relays messages to physician as appropriate. Provide follow-up to patients/ family members/physicians as necessary.
· Documents appropriate information in medical records clearly and accurately according to procedures.
· Identifies patient’s need for education, additional information, etc., and seeks assistance as necessary.
· Follows infection control policies and procedures.
Participates as a team member in the office to facilitate a pleasant cohesive atmosphere for staff, patients, physicians.
The Patient Care Navigator will manage the care coordination for a panel of identified CKD Stage 4 and 5 patients in each office. This position will work in a collaborative manner with the care team in each office to develop an individualized and comprehensive care plan for these patients.
Qualifications & Requirements:
- Current Texas State License
- Preferred - Bilingual
- Previous nephrology experience preferred
Compensation & Benefits:
- Hourly rate dependent upon experience and credentialing
- Full-time (40 hours/ week)
- Monday-Friday, 8am-5pm (No evenings or weekends)
