What are the responsibilities and job description for the Community Health Worker position at Easter Seals - Michigan, Inc?
All persons working within Easterseals are required to conduct themselves in a professional manner at all times and comply with the standards set forth in the Employee Handbook. All employees are required to support the mission of Easterseals in all activities and act as ambassadors.
Community Health Worker will work to promote, maintain and improve the health of persons served. This position will work closely with medical providers, care teams, and other agencies to improve care outcomes. They will facilitate access to community services by connecting and helping persons served navigate systems of care. Specific duties may include:
- Establishes trusting relationships with persons served as they work collaboratively and effectively with clinical care teams.
- Provides ongoing interviewing, goal setting and follow-up for persons served.
- Effectively communicates Easterseals’ mission in establishing relationships with medical staff and persons served.
- Acts in advocacy and liaison role between persons served and community service agencies.
- Assists persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services.
- Connects persons served with transportation resources when appropriate.
- Assists with the completion of required information such as enrollment, eligibility determination and overall follow-up. Assist in the completion of applications and registration forms when needed in order to expedite care.
- Records and tracks care management information. Assesses information to identify patterns and/or trends.
- Maintains knowledge of community resources to assist in meeting the needs of persons served.
- Provides referrals for community services as appropriate.
- Assists persons served in the development of health management plans and personal care goals. Provides coaching in the effective management of their chronic health conditions and self care. Motivates individuals to be active and engaged in their health care.
- Attends regular staff meetings, trainings and other meetings as required.
- Runs and reviews reports and monthly stats for management review.
- Assists in the coordination of Outreach activities.
