Eastern Nephrology Associates is seeking a Chronic Care Management RN to lead outreach and care coordination for patients with chronic kidney disease and related conditions. This role blends telephonic care, one-on-one patient engagement, care plan development, and close collaboration with our nephrology team.
This role is ideal for RNs with prior CCM, home health, hospice, or nephrology experience who thrive in autonomous roles with meaningful patient interaction. Hybrid and remote work options are available for experienced candidates.
We're looking for RNs with a strong foundation in chronic care management or nephrology who are confident working independently and managing a caseload. Remote flexibility may be considered for candidates with advanced experience after successful orientation and proven proficiency.
🔹 What You’ll Do
• Manage a CCM caseload and carry out individualized nephrology-focused care plans
• Collaborate with physicians, APPs, and care teams to ensure continuity of care
• Educate patients on disease management, treatment goals, and self-care strategies
• Coordinate referrals and community resources to remove barriers to care
• Use telephonic and virtual tools to engage patients and document care activities
✅ What We’re Looking For
• RN licensed in NC or SC
• 2 years in chronic care coordination, case management, or nephrology strongly preferred
• Excellent communication, clinical judgment, and follow-through
• Proficiency in EMR systems (EPIC a plus)
• Comfortable using video/audio communication tools for virtual patient interaction
📍 This position can be based in Greenville or Wilmington, NC. Experienced CCM RNs may qualify for hybrid or fully remote work options based on performance and program needs.
