Eastern Nephrology Associates is hiring a Chronic Care Management RN to join our team focused on supporting patients with chronic kidney disease and other complex conditions. This position combines telephonic care, one-on-one patient engagement, care plan development, and collaboration with our interdisciplinary care team.
This is a great opportunity for an RN who enjoys building relationships with patients, working independently, and contributing to meaningful health outcomes. Nephrology experience is a plus, but not required — we’ll support the right candidate in growing into the role.
🔹 What You’ll Do
• Manage a panel of patients enrolled in the Chronic Care Management program
• Develop and implement individualized care plans
• Support patient education and self-management for chronic conditions
• Coordinate referrals and connect patients with community resources
• Engage patients using phone, video, and electronic health record tools
• Collaborate with physicians, APPs, and other care team members
✅ What We’re Looking For
• RN licensed in NC or SC
• Strong communication and problem-solving skills
• Self-motivated with a patient-first mindset
• Knowledge of care coordination and chronic disease management
• Familiarity with EMRs (EPIC preferred) and Microsoft Office tools
📍 This position is based in-office. Hybrid flexibility may be considered over time, based on demonstrated proficiency and the needs of the program.
