Job Title: Case Manager
Program: Annex - Community Support Services
Location: The Annex, The Dalles (with potential work in Wasco, Hood River, and Sherman Counties)
Hours: Tuesday-Saturday, 8:00 am – 5:00 pm, 1.0 FTE (40 hours)
Pay Range:
- Case Manager I: $27.28-$33.16/hour
- Case Manager II: $31.54-$38.34/hour
Overview:
The Case Manager provides case management and housing support services, focusing on helping houseless individuals achieve self-sufficiency and stable housing. Working with MCCFL and Mid-Columbia Community Action Council (MCCAC) at the Annex shelter, the Case Manager builds relationships with clients facing behavioral health challenges, connecting them to services such as behavioral health support, job training, and housing resources.
Responsibilities:
- Assess client eligibility and connect them to services.
- Develop case plans in collaboration with MCCFL and MCCAC.
- Build trusting relationships with shelter guests to address behavioral health barriers.
- Provide transportation and coordinate with community partners.
- Maintain accurate client records and meet documentation standards.
- Conduct housing stabilization work using trauma-informed, culturally competent approaches.
Qualifications:
- Case Manager I:
- QMHA certification or registration within 14 days of hire.
- Preferred: 2 years in human services; experience with severe mental illness; bilingual Spanish/English.
- Bachelor's Degree.
- Case Manager II:
- QMHP certification or registration within 14 days of hire.
- Master’s degree in a qualifying field.
- Valid driver’s license, insurability, and passing a criminal background check.
