Position Objective: The Care Coordinator ensures seamless coordination of care across the Bloomington and Peoria locations. This role oversees referral and intake review, attendance compliance, discharge outcome management, consent tracking, and community resource connections. The Care Coordinator collaborates closely with the Client Access, Care Coordination, and Clinical teams to enhance the client experience and ensure efficient service delivery.
Job Functions:

Referral & Intake Coordination
- Review incoming referrals for completeness (e.g., signatures, diagnoses, prescribed services).
- Follow up with referring providers to obtain missing documentation.
- Manage internal referrals and ensure appropriate routing.
- Flag necessary alerts in the Electronic Medical Record (EMR) and communicate with the clinical team.
- Verify and document all relevant client intake information, sharing it with appropriate departments (e.g., Medical Records, Client Financial Services).

Client Attendance Compliance & Follow-Up
- Monitor client attendance and enforce the attendance policy.
- Communicate with families after no-shows or cancellations, providing education on policy expectations and scheduling tools.

Discharge & Outcome Management:
- Maintain and distribute monthly discharge outcome reports.
- Ensure relevant information is tracked and shared for follow-up.

Consent & Documentation Management:
- Ensure all necessary consents are obtained when coordinating with external providers (e.g., schools, hospitals).
- Regularly review and update consent records in the system.
- Maintain accurate case notes, documentation, and electronic medical records.

Community Resource Coordination:
- Identify families in need of additional resources and facilitate referrals.
- Connect families with funding sources, community organizations, and supportive services.

Communication & Collaboration:
- Serve as a liaison between clients, therapists, and staff to ensure timely care coordination.
- Work with the Scheduling team to align client appointments with service availability.

Administrative Support:
- Assist with coordinating multidisciplinary care teams, including Occupational Therapy, Physical Therapy, Speech Therapy, and other programs.
- Provide backup support to the Care Coordination and Client Access team.
- Assist with special projects as needed.
Marginal Responsibilities:
- Support the training and onboarding of new staff in areas related to patient intake and care coordination.
- Perform other duties as assigned.
Education/Experience Requirements
- Bachelor’s degree in Psychology, Social Work, Child Development, or a related field preferred.
- Minimum of two years of experience working with families or in a medical office, preferably with individuals with special needs.
Competency Requirements
- Proficiency in Microsoft Office Suite and Electronic Health Records (EHR) systems.
- Strong understanding of evaluation, treatment, and discharge workflows.
- In-depth knowledge of community resources and how to access them.
- Familiarity with attendance, discharge, and client consent policies.
Physical Requirements

- Must have visual ability sufficient to read and complete written correspondence, reports, etc., and computer screen information.
- Frequent use of hands to grasp, handle, or feel.
- Regular standing, walking, sitting, and reaching.
- Ability to lift and/or carry up to twenty-five (25) pounds.
- Ability to drive to all Easterseals centers and service sites within a 100-mile radius, as needed.
Work Environment
- Routinely works in office environments with little exposure to excessive noise, dust, and temperature.
- Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Compensation and Benefits:
Expected pay for this position is an annual salary of $40,000 - $45,000. Actual pay will be determined by experience, skills and internal equity.
