The Outreach Engagement Coordinator plays a critical role in fostering engagement with individuals served, ensuring that individuals receive the support they need to actively participate in their treatment and recovery process. In this role, an Outreach Engagement Coordinator will work close with multidisciplinary teams and various departments to ensure individuals are engaged in services and to increase continuity of care across multi-functional teams. Additionally, this position is essential for improving individual outcomes, particularly in the behavioral health setting where engagement is key to effective treatment and long-term recovery.

Qualifications:
- Must be a QMHP in accordance with Medicaid Provider Manual Guidelines. Must meet one of the following:
- Possess specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND
- Be a human services professional with at least a bachelor’s degree in a human services field OR,
- Possess a Bachelor's degree and be an LLBSW/LBSW, or eligible for LBSW with the State of Michigan.
- Community engagement/outreach experience in a human services setting preferred.
- Valid motor vehicle operator license in the State of Michigan.
Duties and Responsibilities:
- Responsible for community outreach throughout the Easterseals MORC geographical area, providing community education and raising awareness of programs and services offered by Easterseals MORC.
- Engage community members in outreach programs to increase their knowledge of Easterseals MORC and available programs and services.
- Proactively reach out to potential and current members, especially those who may be reluctant to seek help or have disengage from treatment. This can include outreach by telephone, telehealth, or in-person visitations to explore barriers to engagement and appointment attendance, develop solutions, provide information, and answer questions.
- Responsible for outreach and engagement activities for new individuals and families who have contacted Easterseals MORC for services or who have been referred to Easterseals MORC for services. Activities may include phone calls, visits, sending letters and scheduling or rescheduling appointments.
- Establish trusting, empathetic relationships with individuals to help them feel supported and motivated to engage in their treatment. This involves listening to their needs, addressing concerns, and creating a safe, non-judgmental environment.
- Help individuals navigate the behavioral healthcare system by providing information about Easterseals MORCs available services, assisting with appointment scheduling, and ensuring individuals know how to access the care they need.
- Work with clinical staff and case managers to ensure individuals receive timely and appropriate care. This may include coordination between departments and ensuring follow-up appointments are scheduled.
- Provide individuals information about their diagnosis and treatment options.
