Minimum Qualifications:
- Must possess a Bachelor's degree from an accredited college or university with a major in Psychology, Social Work, or related mental health field
- Must possess a valid Michigan motor vehicle operator's or chauffeur's license and proof of Auto Insurance
- Must maintain 24 documented training hours relative to Child Diagnostic Treatment Services annually (within any 12- month period).
- Must be MichiCANS certified within the first 3 months of employment and maintain ongoing certification.
Essential Job Functions:
- Functions as the pathway coordinator by assisting with outreach, coordination and screening for children/families referred to a pathway of care.
- Assists with outreach to families to explain services provided.
- Provides appropriate screening for pathways indicated by intake.
- Collaborates and serves as an integral part of the clinical pathway team-based care model.
- Initiates and monitors National Outcomes Measurement Survey (NOMS) to those assigned to care pathway.
- Outreaches to families to support continued screening or outcomes.
- Demonstrates knowledge of principles of trauma informed care, suicide risk and substance use.
- Willingness to support youth and families with varying degrees of high risk of suicide, substance use, eating disorders and psychological trauma.
