Job Summary
We are seeking a highly skilled Clinical Care Manager to join our team. In this role, you will be responsible for assessing and coordinating care and support services for adults with mental illness and children with emotional disturbance and their family who are experiencing crisis. This role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

To be successful in this role, you must have an LMSW, LLMSW, LLPC, or LPC from the State of Michigan Department of Consumer and Industry Services. You must also meet qualifications of a Child Mental Health Professional (CMHP) and/or Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualifications per Medicaid Services.

- Provide case management services, including coordination of care and support services.

- Maintain accurate and up-to-date electronic records, including required documentation of services provided and progress notes.
