Job Description:
We are looking for a Licensed Practical Nurse (LPN) In the Rochester, NH Area to join our team! In this role, you will work as a valued member of our Homemakers and Health Services division to provide physical care and carry out therapeutic regimens for seniors in a home-based setting. Essential Responsibilities in this role include:
- Assist with providing direction and oversight for the medical component of the individuals served consistent with state regulation.
- Help coordinate and/or deliver all nursing care procedures and related health care activities for assigned program.
- Maintain proper documentation for medication management, treatment and assessment, and provide required reports as needed.
- Maintain ongoing, cooperative communication and collaboration with ancillary service providers and physicians.
- Implement quality assurance and oversight of medication management and nursing activities.
- Maintain ample medical and first aid supplies, adequate medications which are accurately labeled and securely store, and a safe and orderly medication area.
- Triage when necessary and, as needed, respond with emergency medical care until appropriate EMS personnel arrive. Prepare incident reports and inform all appropriate parties as required.
- Be a resource to program physicians and assist in all clinical and administration activities related to medical services provided, as required.
- Obtain needed medical, clinical and dental records and treatment as assigned.
- Assist nursing staff and the clinical team in the development, implementation, monitoring and ongoing evaluation of the medical components contained in the treatment plan.
- Help train and supervise staff in relation to health safety and personal care to be provided to individuals.
- Participate in staff meetings as necessary and act as a nurse consultant when requested.
- Ensure and maintain the cleanliness and safety of program areas and follow safety and infection control policies at all times.
- Maintain current state nursing license and required program certifications.
- Provide on-call emergency coverage as required.
Qualifications:
- Licensed Practical Nurse with current NH State LPN License
- Valid Driver’s License and reliable Transportation
Compensation: $33.00 per hour
Hours: Part-Time, 16 hours per week, flexible schedule.
