Easterseals PORT Health is looking for a compassionate and dedicated full-time Family Navigator to join our Individual Community Services (ICS) Team. What You’ll Do
As a Family Navigator on our ICS team, you will play a crucial role in helping individuals and families navigate available services, promoting independence, and fostering a positive, supportive environment. This position involves building structured, strength-based relationships and ensuring best practices, compliance with state, federal, and agency standards, and supporting the personal goals of the individuals and families we serve in the Guilford and Randolph Counties.

You’ll earn a competitive pay of $18.27 plus enjoy a Monday-Friday, 8:30am-5 pm work schedule with some evening flexibility for program and client needs. This role is remote with in-person trainings. What We’re Looking For
To join our team as a Family Navigator, you’ll provide support, resources, and guidance to help individuals and families with disabilities and developmental challenges thrive in their homes, schools, and communities. Have working knowledge of Microsoft office suite, experience with Electronic Health Records software, and have a passion for helping others. We also require the following:
- You’ve had experience caring for someone with intellectual or developmental disabilities (IDD) or you have IDD yourself.
- You have a High School diploma (or equivalent).
- You’re a great communicator and enjoy helping others.
- You have a valid NC driver’s license, a clean driving record, and reliable transportation.
