What You’ll Do
The REACH Admission/Discharge Coordinator plays a key role in managing the admission and discharge process for the Adult Transition Home (ATH), ensuring smooth transitions for individuals entering or leaving the program. Primary responsibilities include coordinating discharge planning, completing assessments, developing individualized service plans, and supporting the overall transition process.

What We’re Looking For
To join our team, you must have experience with crisis prevention and intervention services, a passion for helping others, and have a valid driver's license, good driving record and current auto insurance.
We also require the following:
- Master's degree in health, psychology or social work
- Licensed or Licensed-eligible preferred but not required, or
- Minimum of a bachelor's degree in human services with a Qualified Mental Health Professional (QMHP)
- 3 years of experience with I/DD and/or mental health populations
