Job summary
Under the supervision of the Program Director, the Hepatitis C Virus (HCV) Nurse will plan, develop, coordinate, monitor and evaluate the response of hepatitis disease in patients of Cooperative health and its service communities. This position functions to control and prevent the spread of Viral Hepatitis among adults.
Essential job functions
- Work within a multidisciplinary team with a primary focus to see that patients infected
with HCV enter into treatment and achieve cure.
- Works in partnership with the treating physicians to help facilitate a quality evaluation
and treatment plan for patients
- Provide case management and education
- Link patients to community and Cooperative Health resources
- Assist in the coordination of pharmacy services
- Provide outreach to patients currently engaged in evaluation and treatment
- Ensure patients are fully immunized
- Coordinates application of medications through patients’ insurance companies
- Facilitates clinic process by scheduling and following-up appointments, overseeing
patient flow, preparing patients for procedures, preparing examination and treatment
rooms and maintaining clinic supplies
- Reviews medical records and extracts, enters and updates information into an automated
data file system. Keeps primary provider informed on patient enrollment
- Ensure program patient efficiency and workflow to produce an excellent patient and
clinician experience
- Collect, manage, and analyze program data to ensure program goals are met
- Assist in the creation and implementation of quality improvement activities
- Assist with the coordination and planning of HCV related community outreach activities
- Provide HCV education and awareness to Cooperative Health employees, community
partner organizations, and the community.
- Will enhance personal knowledge through attending conferences and continuing
education events
- Other duties as assigned.
Qualifications
- MSN (master's) preferred but BSN is acceptable
- Must hold current licensure as a registered nurse in South Carolina
- Prior experience with Hepatitis C is not necessary. Must have a strong interest in learning
about the etiology, treatment, and prevention of HCV.
- Relevant work experience may be substituted for education on a year for year basis.
- Current CPR/BLS certification.
- Have the ability to work with patients and physicians from a variety of backgrounds and
lifestyles while maintaining non-judgmental attitude.
- Strong proficiency and experience in developing, implementing and evaluating data and
data management on Excel or other evaluation programs.
- Experience in the use of electronic health records
- Ability to communicate effectively utilizing both oral and written means
- Must possess a current driver’s license.
