Join our compassionate care team as an Assisted Living Nurse Case Manager!
Primary Job Purpose: To address the medical, social, psychosocial, and mental health needs of the neighbors and to enhance their quality of life in the Assisted Living.
Customer Service: Adherence to the MOSES program to include Mission, Ownership, Success, Excellence, and Services.
Essential Job Functions
- Practices the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.
- Responsible for assessing and monitoring neighbor's medical, psychosocial, financial, and mental health needs and reporting to proper agencies and individuals as necessary.
- Responsible for completing required reports related to neighbor's medical, psychosocial, financial, and mental health.
- Responsible for hiring, training, mentoring, and performance management of AL QMAPs and
- Will oversee medication administration of QMAP’s at both The Dahms-Talton Building & Arbor and provide necessary training as needed.
- Will serve and assist neighbors and families with mood and psychosocial needs, specifically in the areas of family and/or resident resolution of issues.
- Will assist with admission screening, admission processes, and discharge planning.
- Will assist neighbors and families with other issues, as needed.
- Will provide accurate and timely documentation and update care plans.
- Responsible for attending psychotropic drug review meetings and presenting necessary information on neighbors.
- Will assist and at times responsible for incident reporting and abuse/neglect investigations.
- Communication of neighbors’ condition & significant issues with physicians, families, staff and Administrator as necessary.
- Will assist the department in survey preparations and regulatory compliance and assist the administrator with quality improvement initiatives.
- Make regular rounds on all neighbors.
- Administers prescribed medications and treatment in accordance with the Nurse Practice Act, with an understanding of the nursing implications (drug actions, interactions, adverse effects, etc.) in terms of patient diagnosis.
- Safeguards and administers drugs in accordance with the law and with established policies.
- Assumes responsibility for personal and professional growth.
- Must maintain confidentiality of all work-related information according to HIPAA regulations.
- Obtain necessary knowledge of State and Federal regulations.
- Participate in developing policies and procedures pertaining to their job roles.
- Assure neighbors/clients' rights, privacy, and HIPAA regulations are followed.
- In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.
Qualifications
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Qualifications, Education, and Experience: Registered Nurse with current license to practice in the state of Colorado. LPN may be considered. Able to supervise QMAP’s. Has the patience needed to work with the needs and demands of neighbors. Healthcare experience in a geriatric setting preferred.
Non-Essential Job Functions: May serve on Eben Ezer committees. May need to assist with neighbors’ transportation on occasion. Will attend facility meetings and department meetings as instructed.

- Experience with long-term care, working with the elderly, and knowledge pertaining to dementia and related issues.
Material and Equipment used: General office equipment, such as phone, fax, copier, adding machine/calculator, and computer. Equipment associated with patient care such as lifts, tubs, wheelchairs, gait belts, beds, blood pressure cuffs, etc.
Working Environment/Physical Activities: Must be able to sit, stand, and walk 8 hours a day. Must be able to lift and carry 0-50 pounds regularly and 50 pounds occasionally with assistance. Must be able to push/pull up to 250 pounds regularly using a wheelchair. Sits, stands, bends, walks, reaches, lifts, keyboarding, pushing wheelchairs, and moves intermittently during working hours. Employee is subject to physically ill neighbors as well as hostile and emotionally upset neighbors and/or families.
Working Hours: As scheduled by the Assisted Living Administrator and which may include evenings, nights, weekends and holidays. Occasional shift work on the floor may be required. Schedule may vary, based on needs and supervisor discretion.
Working Location: This is not a remote position and work must be performed in person.

Salary : $56,485 - $83,500
