What are the responsibilities and job description for the Case Manager - Supportive Housing position at ECHO Housing Corporation?
The purpose of this position is to provide ongoing supportive services and intensive case management to formerly homeless clients enrolled in EHC Permanent Supportive Housing programs via the Housing First Practice Model. Maintaining appropriate levels of practice competency, including an in-depth knowledge of the Housing First model, is an ongoing requirement of this position.
Provide intensive case management services to EHC clients with the goal of helping them to maintain permanent housing and prevent recidivism into homelessness
Meet with clients regularly and conduct ongoing evaluation and assessment to identify specific client barriers and needs
Maintain active and up-to-date goal plans for each client, revisit goal plan regularly with the client and update goals as needed
Connect clients with community resources
Maintain accurate and up-to-date documentation, including timely data entry into HMIS and complete client hard files
Monitor client housing via home-visits and provide assistance, including outside referrals when necessary, to help client maintain home in a stable and secure manner
Job Type: Full-time
Pay: $40,000.00 - $42,000.00 per year
