DISTINGUISHING FEATURES OF THE CLASS : The work involves supervising and coordinating social work, mental health counseling and discharge planning care in Behavioral Health areas at the Erie County Medical Center Corporation. The incumbent develops and oversees a coordinated approach to social work, mental health counseling and discharge planning care as performed by lower-level staff. The incumbent serves as a resource to staff and patients on related matters. Work is performed under the general supervision of a higher-level licensed or administrative employee. Supervision is exercised over lower-level licensed Social Workers / Mental Health Counselors, non-licensed Discharge Planners / Social Work Assistants and clerical staff. Does related work as required.
TYPICAL WORK ACTIVITIES :
- Supervises and coordinates the performance of social work, mental health counseling and discharge planning in a behavioral healthcare setting as performed by lower-level staff and oversees such programs for compliance with applicable laws, rules, regulations and accreditation standards;
- Reviews assessments and services plans; adjusts plans as necessary;
- Assists in the development and implementation of applicable policies and procedures;
- Assists in the evaluation of applicable policies and procedures and makes recommendations for changes;
- Assists with developing departmental goals, supporting projects and cost effective methods for improving patient care;
- Recruits, hires and trains lower-level social work, mental health counseling, discharge planning and clerical staff; prepares performance expectations and evaluates work performance; counsels staff on work performance;
- Identifies training and educational needs of staff; assists staff to develop and improve work skills;
- Plans, schedules and holds individual and group supervision sessions;
- Serves as a resource person; provides counseling and intervention related to post-acute options, treatment decisions, end-of-life issues, resources and related issues;
- Participates in the development of community resources and services;
- Serves as the liaison between other departments, agencies or organizations;
- Provides consultation to lower-level personnel for complex cases and discharges;
- Collaborates with applicable personnel to monitor and review patient throughput and quality improvement / assurance issues; develops recommendations for corrective actions;
- Participates in committees, projects, staff development programs and meetings;
Prepares correspondence and reports and maintains various records.
FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS : Thorough knowledge of the principles, practices and terminology associated with social work, mental health counseling and discharge planning in general and as practiced in a healthcare setting; thorough knowledge of applicable laws, rules, regulations and accreditation standards as they relate to social work, mental health counseling and discharge planning in general and as practiced in a healthcare setting; thorough knowledge of the principles and practices associated with individual and group supervision sessions; thorough knowledge of the clinical assessment principles and practices; good knowledge of available applicable community resources; ability to coordinate social work, mental health counseling and discharge planning care programs in a healthcare setting; ability to plan, supervise and evaluate the work of lower-level staff; ability to work with and assist lower-level staff in developing work skills; ability to review patient throughput for efficiency / effectiveness and develop corrective actions; ability to prioritize and process multiple tasks; ability to prepare clear and concise reports; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodations.
MINIMUM QUALIFICATIONS :
Possession of a Master's Degree
SPECIAL REQUIREMENTS :
Possession of a license and current registration as a Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) in New York State at time of appointment and maintenance throughout duration of appointment; and
Possession of Basic Life Support (BLS) Certification from an ECMCC approved provider at time of appointment and maintenance throughout duration of appointment; and
Section 424-A of the Social Services Law requires that local social services district to inquire whether the applicant is the subject of an indicated child abuse or maltreatment report on file with the State Central Register of Child Abuse and Maltreatment. All potential employees for this position will be requested to sign the necessary clearance form prior to being advised that they will be hired. Refusal to sign will be cause for automatic non-selection.
ADDITIONAL SPECIAL REQUIREMENT IF ASSIGNED TO INPATIENT BEHAVIORAL HEALTH :
Completion of New York State Office of Mental Health-Bureau of Education and Workforce Development (BEWD) certified safety program within three (3) months of appointment and participation in annual re-training.

NOTE 2 : Verifiable part-time and / or volunteer experience will be pro-rated toward meeting full-time experience requirements.
